Claims

Step 1

Contact our claims department at Yourcover on 07 3010 9746 and we will mail or email a copy of the relevant claim form.

Step 2

Please complete the following sections of the claim form:

• Claimant Section A (you as a claimant to complete)
• Doctor Section B (doctor to complete) generally your usual doctor is the best place to have this completed
• Employer Section C (Employer to complete) if you are unemployed your last place of employment

Step 3

Please ensure you have completed the claim form in full. If you need help completing the form do not hesitate to contact our claims department.

Step 4

Post or email the claim form and additional supporting documentation. Original tax forms need to be posted for our clients where PAYG services are provided.
